If you know your PRS organization code and username (this information was
sent to you in an email message when your account was established),
use the form below to request that your password be automatically reset.
Note that the organization code, username and email address must match
the ClinicalTrials.gov information for your account.
An email message containing the new password will be sent to that email address.
If you do not have all of the information required above,
your organization's PRS Administrator can look up your account information
and can make updates if necessary, as well as resetting your password.
If you do not have a local PRS Administrator, you can
send email to firstname.lastname@example.org,
requesting that your password be reset.
